ASSUMPTION OF RISK: All exercises including the use of weights and use of any and all machinery, equipment, and apparatus designed for exercising/martial arts training have certain amount of risk of injury associated with it. These exercises have varying degrees of danger depending on the activity. The risk is minimal if the techniques are carried out correctly. However, if the techniques for the exercises are not executed correctly, then there is a risk associated with these exercises. Some activities such as “sparring” either in Tae Kwon Do, Hapkido, or Kenpo does have a greater degree of risk of injury compared to techniques or workout classes. I accept all risk associated with these activities freely and voluntarily.

WAIVER AND RELEASE OF LIABILITY: Knockout Martial Arts & Fitness urges you and all members to obtain a physical examination from a medical doctor prior to beginning any exercise / martial arts program. Participation in exercises/ martial arts classes shall be at student/member’s risk. Student/member understands that agreement to participate in exercise/ martial arts classes, methods and types of equipment shall be student/ member’s entire responsibly and Knockout Martial Arts shall not be liable to the student/ member for any claims, demands, injuries, damages, or actions, arising due to injury to student/member’s person or property arising out of or in connection with the use by student/ member of the services, facilities, and premises of the school. Student/ member hereby holds Knockout Martial Arts & Fitness , it’s officers, owners, agents, and employees harmless from all claims which may be brought against them by student/ member or on student/ members behalf for any such injuries or claims. I understand and accept all risk associated with these activities freely and voluntarily.
